Support Coordination is support that brings your NDIS plan to life and links you to the supports you require to live the life you want.

  • Understand the Plan;
  •  Connect with Supports and Services;
  • Design Support Approaches;
  • Establish Supports;
  • Coach, Refine, Reflect; 
  • Targeted Support Coordination; 
  • Crisis: Planning, Prevention, Mitigation and Action; 
  • Build Capacity and Resilience; and
  • Report to the NDIA.

Outreach Appointments

Services are by appointment and can be completed by outreach at your home or community center.  Some appointments can be completed over phone or video call and zoom. 



Service Area

The service area covers Bendigo and Greater Bendigo Region. This also includes Castlemaine.

Support can be arranged to provide outreach to Echuca and Shepparton.  This will depend on demand and viability.

Specialise Support Coordination

 Specialised Support Coordination is  utilising an expert or specialist approach, necessitated by specific high complex needs or high level risks in a participant’s situation. Specialist Support Coordination is delivered by an appropriately qualified and experienced practitioner to meet the individual needs of the participant’s circumstances .

  

  • Understand the Plan;

  • Design Support Approaches;

  • Targeted Support Coordination; 
  • Crisis: Planning, Prevention, Mitigation and Action;
  • Address Complex Barriers;
  • Design Complex Service Plan;
  • Build Capacity and Resilience; and
  • Report to the NDIA.

Psychosocial Recovery Coaching

Specialise Support Coordination

  

Psychosocial Recovery coaching is mental health support for participants to build capacity and resilience. This is done through strong and respectful relationships to support people with a mental health disability to live a full and contributing life. 

This support is designed to 

  • Maintain engagement through periods of increased support needs when unwell.
  •  Recovery coaches work collaboratively with participants, families, carers and other services.
  •  Identify, plan, design and coordinate NDIS supports. 
  • The work of psychosocial recovery coaches requires lived and/or learnt experience. 
  • Recovery coaches must have tertiary qualifications in peer work or mental health
